Healthy Age Friendly Homes Programme
Cork County Council and Age Friendly Cork are proud to announce their support for the Healthy Age Friendly Homes Programme. This is a Sláintecare and Age Friendly Ireland joint programme. The initial two-year pilot programmes (2021 to 2023) was successful and approval was granted for the programme to be rolled nationally. The programme is funded by Sláintecare and aims to reach over 600 participants per year in Cork County.
The goal is to support older adults to remain living in their own homes and communities. These older people will have been identified as having significant social care needs and at risk of hospitalisation or premature entry into long term residential care. It is the aim of this programme to target supports at these older people in the form of improving their living conditions, access to continued health and social care supports and tailored community supports. The Local Coordinators will support older people and their families to navigate the services by proactively linking them to the appropriate supports and services.
Two new Local Coordinators have been appointed for County Cork and will be carrying out assessments with the participants. The Healthy Age Friendly Homes Brochure, and Healthy Age Friendly Homes Referral form are available for downloading. Referrals can be made by anyone once consent of the participants has been received or the participants can self refer to the programme if they wish. This is a free service for people age 65 or older.
